Dawn is a Kentucky born certified Family Nurse Practitioner with 25 years of healthcare experience. She is dedicated to providing quality patient centered compassionate care.

How to obtain a card
Obtaining a medical cannabis card is the first step in your medical Cannabis treatment plan. Bluegrass Cannabis Care is committed to simplifying the process of obtaining a card and to helping you achieve the best possible outcome and a healthier you by offering comprehensive, confidential treatment plans and regular follow-up. We will set goals and monitor progress to help You achieve your healthcare goals.

You must currently reside in Kentucky and have a qualifying condition.
Unsure if you qualify, schedule a consultation to determine your eligibility.

Schedule an appointment with Bluegrass Cannabis Care for an in-person consultation - only $150 for your initial visit.
Most often we are able to help patients qualify the same day. We will also structure individualized, confidential treatment plans and goals at this same visit.

Once a certificate of Eligibility has been submitted to the state of KY by Bluegrass Cannabis Care, you will then submit your card application via the state of KY website.
This card application process is simple and quick.
